The New 1500 Health Insurance Claim Form - CMS 1500
CMS (Centers for Medicare and Medicaid Services) has announced the adoption of a new version of the 1500 claim form (CMS-1500 or HCFA-1500). The new form will accommodate the use of the National Provider Identifier (NPI) as mandated by HIPAA.
Changes
Changes to the new form include splitting of box 17a to provide for
reporting of the NPI number or other identifying number. Similar changes were
made to box 24I and box 33. A detailed listing of changes to the
