Registration Request for customers without a current maintenance agreement and for unsupported DTI software versions* 

Please use this form to submit a request for registration if you do not have a current maintenance agreement or if you are re-installing an unsupported DocuTrac Product or installing an unsupported DocuTrac product on a new system.

All field are required for submission. We cannot process requests if fields are omitted.

Depending on the Product selected above, please enter one of the following sets of numbers.

For QuicDoc, go to Preferences...Register QuicDoc and provide the following:

Code Number

Registration Number


For Office Therapy, from the Splash screen - trial meter, click Register and provider the following:
Registration ID

Registratation Key



Be sure to click Cancel after getting the code numbers. This will save them so that the registration code assigned will match the codes you sent.
After you receive a response email with your registration code, return to the Registration screen and enter the code provided to you.

Your request will be processed, if valid, in 24 hours. If submitted after 3pm EST on Friday, a registration request will not be processed until Monday.

*Note: We can no longer provide registration/unlock codes for versions of QuicDoc earlier than V3.86 or Office Therapy earlier than 3.59.
