The Support form for QuicDoc® and Office Therapy® is an on-line form. Using the form is an easy way to get answers to your questions, and it provide us with important information we need in order to assist you.
Support Forms are generally answered by the end of the next business day.

In order to File Claims Electronically and to process Credit Cards you are required to be current on your Annual Maintenance. We cannot assist with claim rejections or credit card errors by any means - phone, fax, email or live help - unless you are current on your Annual Maintenance.
