Description
The HSQ 2.0 is a health outcome instrument designed to measure physical and social functioning, and emotional well-being. The HSQ includes a simple index of health status change and a depression screener. It consists of thirty nine separate questions assessing eight specific health attributes grouped under three dimensions:
1. | Overall evaluation of health: health perception. |
2. | Functional status: physical functioning, social functioning, role limitations attributed to emotional problems. |
3. | Well-being: bodily pain, mental health, and energy/fatigue. |
HSQ items are grouped into eight scales:

Use the HSQ to monitor progress across treatment sessions. The HSQ is also useful for treatment and discharge planning, and for feedback to patients on their progress. The HSQ can be completed in less than 10 minutes, and can be administered to persons 14 years or older.
Scale scores are converted to T-Scores (Mean = 50; SAD. = 10) based on reference data from 1,780 persons aged 65 and older and 2,379 persons ages 18-64. Lower T-Scores represent more positive health attributes. Generally, a T-Score of one or more standard deviation above the mean (>= 60) is significant. Change scores of one or more standard deviations is significant.
