The information entered in the Patient Information window is contained in the Facesheet. You can enter the information at setup or any time after the client has been added to the database. First and last names and the Provider is all that is necessary to add a patient to the database.
Print blank facesheet for your client to fill out
1. | Click the Print button on the toolbar or select File... Print... Measures, Forms, Letters. |
2. | In the Open window, select Facesheet.rtf from the QuicDoc Documents folder. |
3. | Click Open. The Facesheet opens in the QuicWord word processor. |
4. | Select File... Print. Print one or more copies. |
5. | If you have made changes, save the document with a different name so that you can keep the original face sheet form. |
Note: Facesheet.rtf is a separate document that is provided for you as a form to give your patients to fill out and not a template used by QuicDoc.
Patient Facesheet
You can include a list of sessions for a Session History and Current Diagnosis on the facesheet.

1. | Select the client from the patient list. |
2. | Select File...Print... Facesheet. |
3. | You can include the Diagnosis and the session or Contact History and Current Diagnosis by checking these options at the top of the Preview window. |
4. | Click the Refresh button after selecting options or un-checking options. |
5. | In the Print Preview window, select Print to print the Facesheet. |

• | Click and drag mouse to move page up or down. Double click to zoom in. Right double click to zoom out. Use right/left arrows to view previous/next pages. Use the drop down menu of pages to view any page. |
Add items to the Facesheet
When you set up User Fields in the Patient Information window, these fields become a part of the data for the Facesheet. The information you enter for each patent will appear on that patient's Facesheet.
Print and Edit Facesheet for Patient to Complete
You can print out and edit the blank Facesheet. You can edit it to include the information you set up in the user fields, Patient Information window.
To open the facesheet in the word processor to edit, select File...Print...Measures, Forms, Letters.
Select the file named Facesheet.rtf in your QuicDoc\Documents folder. Edit the facesheet in QuicWord, if applicable, to include your user defined fields. Then select Save as... from the File menu to rename and save it.
Note: The blank face sheet is not a template for the Facesheet. It is a separate document that is provided for you as a form to give your patients.
To print, select Print from the file menu in QuicWord.
