Disclosure PHI Log

The Disclosure of Protected Health Information log is designed to track all requests and disclosures of client protected health information.

To open the log from the Patient window, go to Documentation...Disclosure PHI.

To enter a new record, click the New button.

 

ellipse button Click the button to the right of the Patient field for a pick list of clients. Select the client.
Enter the date of disclosure. This field defaults to the current date. It must be filled in order to save a record.
Enter the date the request was received.
Click the checkbox if the request was in writing or was an authorization. If not, use the text field below to enter how the request was received.
Enter the name of the recipient of the information, and agency if applicable.
Enter the address, if known.
Enter a description of what was closed.
Enter the purpose of the disclosure.
Enter the person responsible for disclosing the information. Select the name from the pick list in the Disclosed by field.
Click Ok to save.

 

Edit

Once a record is saved it is listed in the log with all information. You can double click on a row to open the record for editing, or select it, and then click the Edit button.

 

Filter

You can filter the log for a specific patient, or specific dates of disclosure.

Type the client's last name, or first initial or few letters of the last name in the Patient field in the Filter section of the Disclosure log. Click the Apply button to filter.

Click the Reset button to list all records.

Click Show Selected Patient to filter the list for the selected patient.

 

Delete

To delete a record select it from the list, and then click the Delete button.
