Physical Exam consists of Vital Signs and Physical Exam Findings.
 

The Physical Exam section can be setup to appear in any type of note. To use this section, it will need to be setup in the Documentation Setup which requires the  Administrative logon. You can add this to any type of note or include it in your own custom note type. You can view a chart or graph of vital signs (blood pressure, respiration, pulse, temperature, weight, BMI) over time if you have charted them on two or more occasions.
 

To view a graph for a patient, first select a patient, and then select History...Vital Signs from the Patient menu.

Vital Signs

You can enter the following for vital signs:

Date and Time
Vitals Obtained By
Blood Pressure and Position (e.g. sitting)
Pulse and Pattern (e.g. regular)
Respiration and Pattern (e.g. labored)
Temperature and Method (e.g. oral)
Weight
Height
BMI (Body Mass Index) - calculated using the following formula:  (Weight/Height x Height)  x 703
