Your QA Reports will only be as accurate and meaningful as the data you have entered. The QA analysis examines variables you have addressed at Intake and at Discharge. Therefore, if you have not entered a diagnosis at Intake with GAF rating, and then selected the Diagnosis screen at discharge to update the GAF, that patient's principal diagnosis and GAF ratings will not appear in the report. Similarly, if you do not address Risk Factors (see Assessment) at Intake and then update them at Discharge, the breakdown of Risk factors by degree will not be complete or accurate. Select Quality Assessment from the Reports menu in the Patients window. The QA Report shows statistics for patients with a documented Intake and Discharge note. The report includes the following:
Statistics listed by Diagnosis Number of Patients by Diagnosis Average Number of Sessions Seen Average Differential GAF
Summary Statistics for All Patients Number of Patients Seen Breakdown by Sex Average Age Breakdown by Age Range Average Time from Initial Contact to First Appointment Risk Factors at Intake and Discharge: Suicide, Physical Violence, Substance Abuse, Psychotic Features, HIV

Where to enter information If you want to generate complete and meaningful QA Reports do a minimum of an Intake and Discharge Note, addressing the following at each point in time: Principal DSM-IV Diagnosis: Diagnosis form at Intake and Discharge GAF Rating on Axis V: Diagnosis form at Intake and Discharge Number of Sessions/Days: Session Information form at Discharge only Age and Sex of Patients and Average Time to First Appointment: Patient Information form at Patient Setup Discharge Type, and Condition: Session Information form at Discharge only Risk Factors: Risks/Strengths at Intake and Discharge
