Score the Patient Satisfaction Survey

From the Assessments menu, select Patient Satisfaction Survey...New.

1.Enter the client's Discharge date or approximate date.
2.Enter the provider. The field will display the name of the logged on user unless the user has all provider access rights.
3.Select a program and insurance or managed care company from the drop down pick list.
This information is used to filter report and graphs.
4.Enter a rating for each item.
5.Click Save to calculate scores. Scores are displayed at the bottom of the form.

 

Note:  Individual survey results can only be printed at the time the scores are calculated.

Click Print to print the survey results with item ratings, total score, and sub scale scores.
