The Intake Note consists of Intake Note Sections, Other Sections, and Psychosocial History.
The Intake Sections include Session Information and the following clinical sections:
You can address as few or as many of the sections as you wish. The symptoms and goals, however, are determined by your entries in the DSM-IV Diagnosis section.
You can add, remove, and change the order of the sections that make up the Intake Note in the Documentation Setup.
