Risks and Strengths can be recorded in Intake and Progress Notes, Treatment Plans, and Discharge Notes. You can also set it up to appear in any order in any note or report. Risks and Strengths consists of three sub-sections: Risk Factors, Strengths/Weaknesses, Leisure and Interests. Risk Factors You can document risk levels in the areas of Suicide, Violence, Substance Abuse, Psychosis, and HIV. You can also add your own risk factors to track. Select risk factor to address by clicking it in the risk factors list. For Suicide and Violence, enter risk severity (e.g. High, Moderate) and type (e.g. Ideation., Plan, etc). For other risk factors, enter risk severity. You can enter a narrative statement in the details text box for each.
Note: In order to track suicide and violence risks for the Quality Assessment Report, you will need to address these risks in the Risks/Strengths section at Intake and at Discharge. Suicide and violence risks are addressed in the section labeled Plan, in the Risk Management tab for Progress Notes. Add your own Risk Factors You can add any number of user-defined risk factors to track. New risk factors are global and apply to all patients. Strengths and Weaknesses Select strengths from the drop-down list, or type in your own, and click OK. Strengths appear in the list below. To Edit list - If you have access rights, click
Weaknesses Enter Weaknesses in the text box provided. Leisure and Interests Use the text boxes provided to enter information on the patient's interests and leisure activities.
